after rebooting and getting dhcp reconnect again, did it then able to upgrade? you can try setting it as static IP if dhcp is the problem.

you can find it in general settings to change it, if i remember correctly the default is one day. but im guessing your problem is related to "allow https" in the authenticate setting, try change it to "block all", otherwise your clients can access facebook/twitter or other https website without "accept".

@roy Did you happen to input non english character in the static dhcp description? I had a client input french chars and it resulted in the same thing.

did you type the "write" command to save configurations? 

in switch configuration page there is voice vlan to configure, maybe if you configure it to vlan1 +OUI of ur IP phones can have qos raised. personally i think sharing voice with normal traffic is way less managable
