USG Flex100H factory reset doesn't reset password

I have a 100H that I cannot log into via console cable.
I have tried debug mode reset and the reset button methods.
When I try and login with admin / 1234 via the console cable it returns "login: pam_setcred failed: Permission denied (6)"
I'm at a loss of what I can do next to revive this device.

Hi @Arvee ,
If you use the RESET button, please make sure that your steps as below:
- Make sure the PWR/SYS LED is on and not blinking.
- Press the RESET button and hold it until the PWR/SYS LED begins to blink. (This usually takes about 7 seconds.)
- Release the RESET button and wait for the 100H to restart.
